ABSTRACT

Visual inspection of ultrasound examination for assessment of echogenicity and echotexture of blood vessel tissues is a technique routinely used in medical practice in humans. However, simple visual inspection can lead to poor quality diagnoses and errors. The use of grayscale histogram (GSH) analysis has proved to be an efficient technique to quantify the region of interest, allowing minimization of image interpretation errors. This study aimed to evaluate and compare the echogenicity and echotexture of the walls of the common carotid arteries of healthy female horses and mules using the GSH technique and correlate them with age, body mass, and vessel diameters. B-mode ultrasound examinations were performed in the left and right common carotid arteries in three regions (cranial, middle, and caudal) in 11 horses and 11 healthy mules. The GSH of the animals showed heterogeneous walls, but did not differentiate statistically between female horses and mules. The Mean variable of the middle right, middle left and caudal right sides showed differences, more significant in the mules. On the middle right side, the Min variable was different, higher in the mules. On the middle and caudal left side, the variables Max and Mode showed higher values in the mules. For the mules, the age factor presented negative correlation with the Mean, Mode,, Mode(Count), and Mode(Count)/Count(%) variables, and the body mass factor presented negative correlation with the Mode, Mean and Max variables. For the female horses, the body mass factor showed positive correlation with the Mean and Mode variables. Echogenicity of the carotid artery walls differed between female horses and mules, whereas echotexture was heterogeneous and statistically similar among the animals. The age and body mass factors inversely influenced the echogenicity of the mules, but were not significant in the female horses, in which only the body mass factor positively influenced echogenicity.(AU)

ABSTRACT

To compare and find diagnostic accuracy of Transvaginal sonography (TVS) over Transabdominalsonography (TAS) in evaluation of adnexal lesions. A cross sectional comparative study of 50 patientsincluding both pre and postmenopausal women who were clinically suspected to have adnexal masswere scanned on Logiq GE C5 Premium ultrasonography unit. TAS was performed with 3.5MHzelectronic macro convex probe followed by TVS with 6.5 MHz end firing electronic TVS probe. TVSwas found to be superior in diagnosing adnexal lesions. In comparison with TAS, TVS diagnosedbenign cystic teratomas (100% versus 40%), endometriomas (90% versus 57.1%), haemorrhagic ovarian cysts (85.7% versus 50%, hydrosalpinx (75% versus 25%), pyosalpinx/tubo-ovarian mass (75%versus 75%), and ectopic pregnancy (100% versus 77.7%). In our study ovarian cancers were betterdetected by TAS due to their large size. TVS was found to be superior in diagnosing adnexal masses ascompared to TAS with more accurate delineation of internal architectural features as wall thicknessand complexity, nodules, septae, papillary projections, internal echoes thereby narrowing the differentials.

ABSTRACT

PURPOSE: This study was conducted to investigate whether the presence of patchy echogenicity in the liver of patients with chronic hepatitis B (CHB) is predictive of liver stiffness. METHODS: A total of 200 CHB patients with and without patchy echogenicity of the liver were assigned to two groups, with 100 patients in each group, and 32 of them underwent liver biopsy. Additionally, 80 healthy subjects, 100 inactive HBV carriers, and 100 patients with decompensated hepatic cirrhosis were assigned to the control groups. Laboratory tests and clinical data were collected, and shear wave velocity (SWV) of the liver was measured for all 480 subjects. RESULTS: The median SWV in patients with a normal liver, inactive hepatitis B virus carriers, CHB patients with and without patchy echogenicity, and decompensated hepatic cirrhosis were 1.07 m/sec, 1.08 m/sec, 1.16 m/sec, 1.16 m/sec, and 2.02 m/sec, respectively; there was no significant difference in SWV values between CHB patients with patchy echogenicity and those without patchy echogenicity. Furthermore, among CHB patients with and without patchy echogenicity, no significant difference in SWV was found according to fibrosis stage. CONCLUSION: The presence of patchy echogenicity of the liver does not indicate a higher degree of liver stiffness.

ABSTRACT

OBJECTIVE: To determine the histopathologic features associated with ultrasonographic echogenicity of thyroid nodules. MATERIALS AND METHODS: This study included 95 nodules of 95 patients (76 women, 19 men; mean age 47.5 ± 12.9 years) with homogeneous echogenicity in which core needle biopsy was performed during a one year period. The nodule echogenicity was categorized into 4 grades (hyperechogenicity, isoechogenicity, mild hypoechogenicity, and marked hypoechogenicity). The biopsy specimens were evaluated by a pathologist regarding the histopathologic features of fibrosis, lymphoid infiltration, microfollicular pattern, uniform follicular pattern, and hypercellularity in nodules. We evaluated the association of each histopathologic feature among 3 categories of nodule echogenicity by multinomial regression analysis. RESULTS: The nodule echogenicity was isoechoic in 28 (29.5%), mildly hypoechoic in 37 (38.9%), and markedly hypoechoic in 30 (31.6%), and there was no hyperechoic nodule. There was a trend of increasing frequency of fibrosis (> 30%) as nodule echogenicity decreased (isoechogenicity, 10.7%; mild hypoechogenicity, 32.4%; and marked hypoechogenicity, 80%; p 30%) and hypercellularity were independently associated with mild or marked hypoechogenicity as compared to isoechogenicity (fibrosis; p ≤ 0.004 and hypercellularity; p ≤ 0.036), and only fibrosis (> 30%) was independently associated with marked hypoechogenicity as compared to mild hypoechogenicity (p = 0.004). CONCLUSION: The fibrosis (> 30%) and high cellularity are independently associated with mild or marked hypoechogenicity of nodules. The knowledge of the relationship of echogenicity and histopathology of thyroid nodules could improve management of patients with thyroid nodules.

ABSTRACT

PURPOSE: Renal ultrasonography has been widely used in children with renal disease. However, the relationship of renal echogenicity with renal pathology and function in children is not well known. METHODS: Ultrasound examination was performed in 75 patients undergoing renal biopsy for suspected renal disease in Konkuk University Medical Center from August 2005 to November 2015. We compared renal echogenicity to pathologic findings and renal function. Renal echogenicity was scored as 0 to 2 by comparing adjacent liver echogenicity. Three histologic characteristics were evaluated: glomerular changes, interstitial infiltration or fibrosis, and tubular atrophy. These were graded as 0 to 3, according to increasing severity. Laboratory results included urine albumin excretion and estimated glomerular filtration rate (eGFR). RESULTS: Among pathologic findings, renal echogenicity revealed a positive correlation with interstitial infiltration or fibrosis (r=0.259, P=0.025), and with tubular atrophy (r=0.268, P=0.02). Renal echogenicity and glomerular changes were not correlated. Renal echogenicity showed a positive correlation with microalbuminuria (r=0.283, P=0.014), but a negative correlation with eGFR (r=-0.352, P=0.002). CONCLUSION: Increased renal echogenicity suggested severe interstitial infiltration or fibrosis and tubular atrophy among the pathologic findings. Moreover, increased echogenicity is correlated with increased urine albumin excretion and decreased eGFR. Echogenicity on ultrasonography is useful for determining the status of renal pathology and function.

ABSTRACT

Ultrasound is a diagnostic imaging method that allows a no invasive evaluation of different organs and body structures. However, the personal evaluation of images is one of the major hampers in using this technique. In order to reduce this drawback, it is necessary to find out tools that can help diminish the divergence in the interpretation of sonographic findings. Therefore, this study aimed to identify features of the parenchyma's and mediastinum's testis of sheep echogenicity using the technique of gray scale histogram. 30 animals were divided into three groups according to age (FE): three to six months (FE1), seven to 12 months (FE2), 13 to 18 months (FE3). The testicular scans were performed in the frontal, sagital and transverse views, and the gray scale histograms were analyzed. It was observed that such parenchymal as the mediastinum testis echogenicity gradually increased with the aging of the animals, with mean and standard deviation of 95.00±19.05 and 94.35±18.82 for the echogenicity parenchyma of the right and left antimere, respectively, and 127.95±12.97 to 126.59±11.78, for the right and the left mediastinum. The technique of gray scale histogram was an efficient method for determination of testicular echogenicity, enabling the establishment of normal standards that may assist future research in monitoring testicular development as well as for detecting pathologies. For exclusive regimes of extensive farming, as the western lowland region of Maranhão, is a valuable tool for use in social projects of the State to attend the family farm.

ABSTRACT

Background: High-frequency ultrasound is a noninvasive tool that offers characteristic markers, quantifying the cutaneous changes of the physiological senescence process. Aims: The aim was to assess the changes in skin thickness, dermal density and echogenicity, as part of the ageing process, with different age intervals. Methods : The study was performed on 160 patients, aged 40.4 ± 21.2, divided into four age categories: <20, 21-40, 41-60, 61-80. Ultrasonographic images (Dermascan device) were taken from three sites: dorsal forearm (DF), medial arm (MA), zygomatic area (ZA). We assessed the thickness of epidermis and dermis (mm), number of low, medium, high echogenicity pixels, the ratio between the echogenicity of the upper and lower dermis (LEPs/LEPi), and SLEB (subepidermal low echogenicity band). The statistical analysis was performed using SPSS 15.00. A P value <0.05 was considered significant. Results: On all examined sites, it was found that the dermal thickness increases in the 21 to 40 year interval (P<0.0001). After the 21 to 40 year interval, the number of low echogenic pixels increases significantly, especially on photoexposed sites. High-echogenic pixels follow the same pattern on all examined sites: they increase in the 21 to 40 year interval and decrease in the 3rd and 4th age category. The LEPs/LEPi ratio increases significantly with age, at all sites (P<0.05), due to an increase of hypoechogenic pixels in the upper dermis. Conclusions: High-frequency ultrasound is a noninvasive "histological" tool that can assess the cutaneous structure and age-related changes. It offers imagistic markers, comparable to the histological parameters and also characteristic ultrasonographic markers. Histology remains the gold standard for the investigation of the integumentary system.

ABSTRACT

ABSTRACT

OBJECTIVES: Transcranial sonography of the substantia nigra for diagnosing premotor stages of Parkinson disease has been attracting increasing interest. Standard reference values defining an abnormal increased echogenic size (hyperechogenicity) of the substantia nigra have been established in several populations using high-end stationary ultrasound systems. It is unknown whether a portable ultrasound system can be appropriately used and how the Filipino population would compare with the well-studied white population. METHODS: We prospectively studied substantia nigra echogenic sizes and third ventricle widths in 71 healthy adult German participants and 30 age- and sex-matched Filipino participants using both a well-established stationary ultrasound system (in the German cohort) and a recently distributed portable ultrasound system (in both ethnic cohorts). RESULTS: Mean substantia nigra echogenic sizes, cutoff values defining abnormal hyperechogenicity, and intra-rater reliability were similar with both systems and in both ethnic cohorts studied. The Filipino and German participants did not differ with respect to the frequency of insufficient insonation conditions (each 3%) and substantia nigra hyperechogenicity (10% versus 9%; P = .80). However, third ventricle widths were smaller in the Filipino than the German participants (mean ± SD, 1.6 ± 1.1 versus 2.4 ± 1.0 mm; P = .004). CONCLUSIONS: The frequency of substantia nigra hyperechogenicity appears to be homogeneous in white and Asian populations. Screening for this feature may well be performed with a present-day portable ultrasound system.

ABSTRACT

This study aimed to evaluate the effectiveness of a computer-assisted ultrasound analysis of senile dogs cataractous lens and identify its correlation to phacoemulsification surgical parameters. Simultaneous A- and B- mode ultrasonic evaluation of the lens was performed on 15 dogs with cataract. A computer-assisted image analysis was used to objectively calculate the lens echogenicity. Following ultrasonographic examination the cataracts were removed by phacoemulsification and the ultrasonographic and phacoemulsification findings were compared to identify possible correlations. Lens echogenicity obtained by the computer-assisted ultrasonographic analysis correlated to the phacoemulsification time, such that the whiter the ultrasound image, the longer was the phacoemulsification time. The B-mode ultrasonography was helpful for the prediction of lens opacity location, but not predictive of lens hardness.

ABSTRACT

We measured changes in sonographic findings of patients with clonorchiasis after a treatment in a highly endemic area. A total of 347 residents showed positive stool results for Clonorchis sinensis eggs in a village in northeastern China, and were treated with praziquantel. Of them, 132 patients underwent abdominal sonography both before and 1 year after treatment, and the changes in sonographic findings of 83 cured subjects were compared. Diffuse dilatation of intrahepatic bile ducts (DDIHD) was found in 82 patients (98.2%) before and 80 (96.4%) after treatment, which was improved in 3, aggravated in 1, and unchanged in 79 patients. Increased periductal echogenicity (IPDE) was observed in 42 patients (50.6%) before and 45 (54.2%) after treatment, which was improved in 5, aggravated in 8, and unchanged in 70 patients. Floating echogenic foci in the gallbladder (FEFGB) was detected in 32 patients (38.6%) before and 17 (20.5%) after treatment, which was improved in 20, aggravated in 5, and unchanged in 58 patients. Improvement of FEFGB only was statistically significantly (P = 0.004). The present results confirm that DDIHD and IPDE persist but FEFGB decreases significantly at 1 year after treatment. In a heavy endemic area, the sonographic finding of FEFGB may suggest active clonorchiasis 1 year after treatment.

ABSTRACT

Chronic autoimmune thyroiditis is currently considered as the main cause for hypothyroidism and its diagnosis is based on clinical manifestations and laboratory tests results. The most significant laboratory marker for this disease is the presence of anti-thyroperoxidase and anti-thyroglobulin antibodies, the latter being the most sensitive one. Aspiration biopsy shows high sensitivity and specificity but, considering the invasiveness of this method it is reserved for cases of suspected nodules or fast growing goiter. Scintigraphy is unnecessary for the diagnosis, considering its low specificity and sensitivity. Both B-mode and duplex color Doppler ultrasonography have rapidly evolved, becoming a simple, noninvasive and reproducible method with high sensitivity for the diagnosis of chronic autoimmune thyroiditis. Echogenicity is a relevant parameter at B-mode ultrasonography because of the close correlation with cytopathological findings besides the high sensitivity in the diagnosis of the disease. Although this is not a specific parameter for chronic autoimmune thyroiditis, considering that it may also be found in Graves' disease, postpartum thyroiditis and subacute thyroiditis, such disorders can be easily differentiated both by clinical-laboratory and duplex color Doppler findings. Thus, the present study is aimed at evaluating the significance of thyroid echogenicity in the diagnosis of chronic autoimmune thyroiditis.

ABSTRACT

PURPOSE: This study was designed to address the correlation between sonography of a kidney with histological lesions and clinical findings in patients with renal parenchymal disease based on a multivariate logistic regression analysis. METHODS: Clinical and laboratory data, sonograms and renal biopsies were evaluated in 154 patients. Cortical echogenicity was graded as less than (0), equal to (1) or greater than (2) liver/spleen parenchyma. Histological lesions - mesangial proliferation (MP), leukocyte permeation (LP), fibrinoid necrosis and crescents (FNC), interstitial infiltrate (II), segmental glomerular sclerosis (SGS), glomerular obsolescence (GO), tubular atrophy (TA) interstitial fibrosis (IF) and interstitial edema (IE) - were graded according to extension and severity as normal (0 percent), mild (<25 percent), moderate (>25 percent <50 percent), and severe (>50 percent). RESULTS: a) II, IF, SGS, IE and increased creatinine occurred less in cortical echogenicity grade 0; b) MP, arterial hypertension and normal parenchymal thickness predict cortical echogenicity grade 1; c) IF, IE, increased creatinine and thin parenchyma predict occurrence of echogenicity grade 2; d) Excluding obese patients, both youth and hematocrit accounted for pyramid prominence; e) increased creatinine and GO was probable in patients with small kidneys. CONCLUSIONS: Increased cortical echogenicity was a very sensitive marker of renal parenchymal disease. Different lesions rather than degree of lesion severity accounted for progressive increase of cortical echogenicity. IE exponentially increased the effect of IF on cortical echogenicity.

BACKGROUND: Coronary atherosclerosis is a pathologic process that produces thickening of the wall of the coronary arteries. but coronary angiography does not examine the wall but only the lumen of the vessels. IVUS has confirmed autopsy data that coronary atheroslerosis is diffuse and almost always involve the proximal portion of coronary arteries. We assess the meaning of the high echogenicity with thickening (HET) on transthoracic coronary echogram of proximal coronary arteries and to evaluate whether HET predicts the severity of coronary artery disease on coronary angiogram. METHODS: 112 patients (M:F=73:39, mean age=57+/-12 years) referred for coronary angiography were included in this study. We detected proximal coronary artery using transthoracic echocardiogram (3.5 MHz transducer, H-P Sonos 5500). We defined that high echogenicity with thickening (HET) is more than 1.5 mm in thickness with high echogenicity and persistence. Coronary angiography were performed in all patients. Of these patients, 29 vessels were examined by IVUS (Intravascular Ultrasound, 30 MHz) and 122 vessels by Fluoroscopy. RESULTS: 1) HET on coronary echogram had sensitivity of 80% and 88% and specificity of 79% and 82% for identifying calcification on IVUS and fluoroscopy, respectively. 2) HET on coronary echogram had sensitivity of 74% and specificity of 86% for identifying the significant stenosis of proximal left coronary artery. 3) HET on coronary echogram was observed more frequently in 3 vessel diseases compared to normal coronary arteries and 1 or 2 vessel diseases (p<0.05, respectively). 4) HET on coronary echogram was observed in vessels with more complex lesions. CONCLUSION: High echogenicity with thickening on coronary echogram may be related to the presence of calcification and predict far advanced coronary atherosclerosis. Technique using echocardiographic tissue characterization of proximal coronary arteries may become a sensitive means of screening for coronary atherosclerosis in asymptomatic patients.
